Lightweight Markdown records (MADR-style) that capture the why of a decision — context, decision, consequences — append-only. The cost is ~30–60 minutes per significant decision; the return is days saved reconstituting lost context later. The discipline is the significance filter: an ADR is not Any Decision Record.
